By entering the 999 3 rd Avenue premises, and/or attending or participating in this event you acknowledge that you may be exposed to and/or may be at risk of contracting or spreading COVID-19. By attending this event, you certify that you currently or within the past ten (10) days have not experienced any symptoms associated with COVID-19, which include fever, cough, and shortness of breath among others. Also, if you believe that you may have been exposed to a confirmed or suspected case of COVID- 19 or have been diagnosed with COVID-19, we ask that you do not attend this event.
Assumption of Risk for COVID-19: I understand that my participation in this event is voluntary and is not required. By signing below, I acknowledge that I have carefully read the above; understand the risks of COVID-19 associated with participating in this event. I voluntarily assume such risks, including the risk of serious illness, debilitating injury, or death. I further acknowledge that I understand that the risk of exposure to, contracting, or spreading COVID-19 may result from the acts, omissions, or negligence of myself and others, including other program participants and their families. I knowingly assume such risks. Waiver of Liability, Indemnity, and Hold Harmless: By signing below, and in consideration of the opportunity to participate in the Activity, I voluntarily agree to waive and discharge any and all claims against SURF Incubator or Leaner Startups related to or arising out of COVID-19, and voluntarily release SURF Incubator or Leaner Startups from liability for any exposure to or illness or injury from COVID-19, including claims for negligent actions of SURF Incubator or Leaner Startups or its employees, agents, representatives, and volunteers related to or arising out of COVID-19, to the fullest extent allowed by law.
