By attending this event, you certify that you currently or within the past ten (10) days have not experienced any symptoms associated with COVID-19, which include fever, cough, and shortness of breath among others. Also, if you believe that you may have been exposed to a confirmed or suspected case of COVID- 19 or have been diagnosed with COVID-19, we ask that you do not attend this event. Assumption of Risk for COVID-19: I understand that my participation in this event is voluntary and is not required. By signing below, I acknowledge that I have carefully read the above; understand the risks of COVID-19 associated with participating in this event. I voluntarily assume such risks, including the risk of serious illness, debilitating injury, or death. I further acknowledge that I understand that the risk of exposure to, contracting, or spreading COVID-19 may result from the acts, omissions, or negligence of myself and others, including other program participants and their families.
